The Tarif 340 is the official tariff framework governing the billing of diagnostic neuropsychological services covered by compulsory health insurance (LAMal) in Switzerland. This guide details the codes, billing rules and caps in force for 2026.
Neuropsychology long operated without a unified tariff convention under LAMal. On 19 March 2021, the Federal Council approved the tariff agreement concluded between H+ Les Hôpitaux de Suisse, the ASNP (Swiss Association of Neuropsychologists), curafutura and tarifsuisse.
This convention came into force on 1 April 2021. It has been extended and remains applicable until 31 December 2026. It is based on Art. 11a OPAS (Ordinance on Health Insurance Benefits), which includes neuropsychological examinations in the catalogue of services reimbursed by health insurers.
Before this convention, neuropsychologists often billed through hospital structures or individual agreements with insurers. Tarif 340 established a clear framework with defined tariff positions and uniform capping rules.
Tarif 340 comprises five positions numbered 100.003 to 100.007. All share the same unit value of 15.36 points per 5-minute period, but each covers a distinct type of service.
| Code | Description | Type | Cap per prescription |
|---|---|---|---|
| 100.003 | Diagnostic examination (patient present) | Present | 72 × 5 min = 6 hours |
| 100.004 | Analysis, scoring, report writing | Absent | 72 × 5 min = 6 hours |
| 100.005 | Feedback interview and counselling | Present | 18 × 5 min = 1 h 30 |
| 100.006 | Telephone consultation | Remote | 24 × 5 min = 2 hours |
| 100.007 | Services in absence (excl. telephone) | Absent | 48 × 5 min = 4 hours |
100.003 — Diagnostic examination
The core of the assessment: administration of standardised and semi-standardised tests, history taking, behavioural observation, exploration of cognitive functions (attention, memory, executive functions, perception, emotional processing). Each 5-minute period spent with the patient is billable under this code.
100.004 — Scoring and report writing
This code covers all work performed in the patient's absence: test correction, score analysis, result interpretation considering history and medical data, and writing the neuropsychological report. The 6-hour cap reflects the real workload, often underestimated.
100.005 — Feedback
The session where the neuropsychologist presents findings to the patient, discusses the diagnosis and formulates recommendations. This code also applies to exchanges with relatives and legal representatives (children, non-communicating patients).
100.006 — Telephone consultation
Telephone calls with the patient or their legal representatives, directly related to the neuropsychological examination.
100.007 — Services in absence (excl. telephone)
This code covers administrative and clinical activities outside the patient's presence and outside telephone calls: gathering information from third parties, coordination with other therapists, file review, written recommendations.
Art. 11a OPAS sets clear limits:
There is no standardised extension procedure (unlike physiotherapy or speech therapy). Any extension requires a justification sent to the insurer's medical advisor, based on the EAE criteria (efficacy, appropriateness, cost-effectiveness) of Art. 32 LAMal.
The medical prescription must be attached to the statement sent to the insurer. OPAS does not set an explicit validity period for neuropsychological prescriptions.
The tariff is calculated using three components:
| Component | Value |
|---|---|
| Points per 5-min period | 15.36 |
| Cantonal point value | 0.98 CHF |
| Amount per 5-min period | 15.36 × 0.98 = 15.05 CHF |
| Effective hourly rate | 15.05 × 12 = 180.60 CHF/h |
The point value of 0.98 CHF has been in force since 2023. It applies uniformly to all Tarif 340 positions. This cantonal factor could theoretically vary, but it is currently identical across all cantons for LAMal neuropsychology.
Neuropsychologists often work with multiple insurance regimes. It is essential to distinguish Tarif 340 from Tarif 330.
| Criterion | Tarif 340 (LAMal) | Tarif 330 (LAA/AI/AM) |
|---|---|---|
| Insurers | LAMal health insurers | SUVA, private LAA, AI, AM |
| Number of positions | 5 (100.003 — 100.007) | 11 (100.001 — 100.011) |
| Therapy covered | No | Yes (100.001, 100.002) |
| Assessments | No | Yes (100.010, 100.011) |
| Point value | 0.98 CHF | 1.00 CHF |
| Points / 5 min (diagnostic) | 15.36 | 15.53 |
| Effective hourly rate | 180.60 CHF/h | 186.36 CHF/h |
| Billing model | Tiers payant | Varies by insurer |
Key point: Tarif 340 covers only diagnostic services. Neuropsychological therapy (cognitive rehabilitation) is not reimbursed under LAMal through this tariff. If a patient requires therapeutic follow-up, it must go through LAA, AI, or out-of-pocket funding.
Who can prescribe a LAMal neuropsychological assessment?
All Swiss physicians with a licence to practise can prescribe a LAMal neuropsychological examination. No medical specialisation is required. A general practitioner can therefore refer a patient for a cognitive assessment.
In practice, prescriptions most commonly come from: neurologists, psychiatrists and geriatricians, general practitioners (suspected cognitive disorders, ADHD, etc.), and rehabilitation physicians (post-stroke, head trauma).
Billing model: tiers payant
The H+/ASNP convention stipulates that the insurer is the debtor. The billing model is therefore tiers payant: the neuropsychologist sends the invoice directly to the health insurer, with no advance payment by the patient.
| Rule | Detail |
|---|---|
| Payment deadline | 30 days (25 days for electronic billing) |
| Billing deadline | 90 days following the service |
| Mandatory format | XML per Forum Datenaustausch |
| Tariff code in invoice | 340 |
Notable exception: Atupri and Assura may refuse tiers payant. In this case, an assignment agreement allows the patient to delegate their reimbursement right to the neuropsychologist.
Can a neuropsychologist bill therapy under Tarif 340?
No. Tarif 340 covers only diagnostic services. Positions 100.001 (individual therapy) and 100.002 (group therapy) only exist in Tarif 330, applicable to LAA, AI and AM insurance. Neuropsychological rehabilitation under LAMal remains a coverage gap.
What if 6 sessions are not enough for a complete assessment?
The prescribing physician can issue a second prescription within the same calendar year, granting 6 additional sessions. Beyond 12 sessions per year, an exceptional coverage request must be submitted to the insurer's medical advisor with solid clinical justification (EAE criteria).
How is the cost of a typical assessment calculated?
A complete adult neuropsychological assessment typically takes 3 to 4 hours of examination (100.003) and 3 to 4 hours of scoring/writing (100.004), plus 30 to 60 minutes of feedback (100.005). At 180.60 CHF/h, an 8-hour assessment totals approximately CHF 1,445.
What is the difference between Tarifs 340, 590 and 581?
Tarif 340 is specific to LAMal neuropsychology. Tarifs 590 and 581 concern psychotherapy and clinical psychology. Tarif 582 is the LAA/AI/AM counterpart of 581. Each tariff has its own positions, point values and capping rules.
The complexity of Tarif 340 lies less in its structure than in its administrative management: tracking caps per position, juggling between tiers payant and assignment agreements, producing XML invoices compliant with Forum Datenaustausch standards, and monitoring the 90-day billing deadline.
Neuropsychologists who manage these constraints manually (spreadsheets, Word documents) spend considerable time on it — time that could be dedicated to their patients.
Therago Neuro is designed to automate this management. The neuropsychology module natively integrates Tarif 340 with its 5 positions, calculates caps in real time, generates invoices in the regulatory XML format and tracks prescriptions per patient. Service entry takes just a few clicks, with automatic overshoot control.
